The center provides a variety of services for adults 50 years and older who live in the Azusa area. Services include information and referral; case management; blood pressure, hearing, and other health screenings; social activities and trips; income tax assistance; Medicare counseling; and recreational/leisure/arts instruction classes. People who are age 60 and older are eligible for case management services. Most services are restricted to residents of Azusa.
Case management services are provided through Santa Anita Family Service. Workers assess the needs of frail seniors and develop, implement, and coordinate a service plan that can help to support seniors in their own homes. The service can continue as long as it is helpful. APPLICATION PROCEDURE Call or walk in for information or for services. Reservations are needed for Dial-A-Ride services. FEES/PAYMENT SOURCE There are fixed fees for classes, some activities and trips. There is no fee for case management.
Hours: Mon-Thurs, 8:30AM-4:00PM; Fri, 8:30AM-1:30PM.
