The agency provides outpatient mental health services for people with chronic/severe mental health issues. Services include individual counseling, family counseling, home based mental health services, and wraparound facilitation/community support. The program is provided at three locations in Montebello, Pasadena and West Covina, however, there are no geographic resrictions.
The program serves children, families and individuals birth to twenty-four years of age, living in the community. Each client must meet the Department of Mental Health medical necessity criteria and be full-scope Medi-Cal eligible. Services offered in the client’s home are targeted to young childen, age 0 to 5, however older chidren and adolescents may be seen in this program. Each client receives a full mental health assessment, individual therapeutic interventions, targeted case management, and medication support services as needed.
The Wraparound facilitation/community support program is provided for court-referred youth in probation or in DCFS child abuse or foster care programs. Wraparound is conducted by a multi-displinary team consisting of a social worker case manager, a therapist and a parent “Partner”. Sessions are conducted in the home and may involve just the child or the whole family as needed. The program accepts youth from age 8 to 18, and may continue for one year or longer. The service area is LA County SPA3 APPLICATION PROCEDURE Call to make an appointment. Referrals are taken from DCFS, Probation, Community based Organizations or people may self refer. Clients must have full scope Medi-Cal. Required documentation includes copies of social security card, Medi-Cal card, government issued identification card or drivers license, proof of income, proof of address, and immunization record. FEES/PAYMENT SOURCE There are no fees to the client.
