The foundation’s clinics provides health services primarily for low-income Los Angeles County residents, including people who are undocumented and individuals who are uninsured. Services include health education, pediatric services, mammograms, pap tests, support groups, and volunteer opportunities. Alternative medicine including chinese herbal medicine and acupuncture, and dental services are also available. There are no geographic restrictions; income eligibility requirements apply for some services.
The clinic generally serves low-income people without health insurance, who meet income eligibility according to the Federal Poverty Guideline Index. Household income cannot exceed $12,000 for a single person; $16,800 for two; $19,200 for three; or $21,000 for a family of four; cash resources may not exceed $2,000 for a single person; $3,000 for two; $3,150 for three; or $3,300 for four. This is subject to change under the Federal Poverty Guidelines.
Chronic disease care is limited to triage and referral; referrals for additional services are available. Mental health and psychiatric services are limited to education and family consultation and do not include hospitalization or medication.
The clinic also provides community outreach and education including health fairs and printed materials. In addition, the clinic provides volunteer opportunities for adults, age 18 and older.
A cancer support group is available for people with cancer and their families. LANGUAGES OTHER THAN ENGLISH Burmese, Cantonese, Mandarin, Spanish, Taiwanese; the South El Monte location can accommodate Cantonese, Mandarin and Spanish. APPLICATION PROCEDURE Call to apply. Photo ID is required for adults, as are proof of income and age; a W-2 or tax return is preferred for proof of income, but check stubs are accepted from part-time workers. A parent or guardian must sign an authorization form for children; social security numbers are requested for all children.
For the South El Monte site, walk-ins are accepted but appointments are preferred; no documentation is needed.
Call for information about volunteer opportunities. FEES/PAYMENT SOURCE There are fixed fees; a $25 registration fee at the health clinics; lab, medication and x-rays are an additional cost. The South El Monte and Wilmington Dental Clinic there is a $25 registration fee and dental treatment fees vary according to services. Doctor’s exam and diagnosis are free.
