The clinic provides health services for people of all ages. Services are targeted, but not restricted, to residents of Altadena, Pasadena and Sierra Madre. Services are provided at five clinic locations. There are no geographic restrictions.
Services include community clinics; contract clinics; CHDP exams; general medical care; physical examinations, cholesterol checks, cancer screening, communicable disease screening, vision and hearing testing, family planning, blood pressure screening, and medications and prescriptions.
Contract clinic services include outpatient health services for uninsured, low-income Los Angeles County residents of all ages, including General Relief recipients. It provides primary general medical care. Applicant’s household income must be at or below 134% of federal poverty guidelines, or who are GR recipients.
CHDP provides regular, preventive health assessments and developmental assessments for children from birth to age 21 who are Medi-Cal eligible and for children from birth through age 18 whose family income is less than 200% of the federal poverty level. The check-ups are offered at set intervals and may include the required Head Start or State Preschool and pre-first grade exams.
DENTAL SERVICES ARE ON HOLD FOR NEW PATIENTS. Dental services include routine cleaning, examinations and restorative work. Dental services do NOT include Root canals, implants and crowns. Denti-Cal and private insurance are accepted.
APPLICATION PROCEDURE Call or walk in to apply; an appointment is preferred.
FEES/PAYMENT SOURCE The clinic accepts private insurance, Medi-Cal, CHDP, Medicare and Family PACT. The clinics have a Covered California certified enrollment intake counselor. For all individuals, fees are set on a sliding fee scale based on ability to pay. No one is refused service due to an inability to pay, but they may receive a bill.
