The center provides health services for people of all ages who live, primarily, in the East San Gabriel Valley.
Services include a general family practice which provides diagnosis, treatment, lab tests, EKG’s, blood pressure checks, physical exams, and general medical care for people of all ages; pediatric services which include well baby clinics, free immunizations, school and sports physicals, and CHDP (Child Health and Disability Prevention) exams; annual exams, treatment of minor infections; tuberculosis screening; sexually transmitted diseases for both men and women; and limited pharmacy services. Health education classes are available to patients including nutrition counseling.
The clinic participates in the L A County contract clinic program. Services provided include outpatient health services for uninsured, low-income Los Angeles County residents of all ages, including General Relief recipients. It provides primary general medical care and dental care. Applicant’s household income must be at or below 134% of federal poverty guidelines, or who are GR recipients.
CHDP provides regular, preventive health assessments and developmental assessments for children from birth to age 21 who are Medi-Cal eligible and for children from birth through age 18 whose family income is less than 200% of the federal poverty level. The check-ups are offered at set intervals and may include the required Head Start or State Preschool and pre-first grade exams.
The center provides outreach education programs in the community including family planning education, HIV prevention and STD and HIV counseling and testing, and teen pregnancy prevention.
Hours: Mon-Thurs, 8:00AM-8:00PM; Fri-Sat, 8:00AM-4:00PM.
