As a Patient Centered Medical Home (PCMH) and Federally Qualified Health Center (FQHC), we have been proven to provide the highest quality care and services that are tailored to each patient’s specific needs.

Medical, Dental and Behavioral Health Services Hours: Tuesday – Wednesday, 8:30am – 5:30pm; Thursday, 9:00am – 5:30 pm; Friday, 8:30am – 5:30 pm; Saturday, 8:00am – 4:30pm; Sunday and Monday Closed
